Title: Puff of smoke and it works!

I had a channel dead so after replacing the triac it was still not working. I figured the triac was ok so I went to cut the leads on the triac but forgot I still had the unit plugged in  <fp.. After a puff of smoke, few sparks and  a loud #%$^&@ I replaced the fuse and everything works including the dead channel. I need a new pair of cutters now.  <fp.

You'll want to keep those triac-fixing cutters around "just in case" you have another dead channel.

Had one of those experiences myself with an SSR that I was closing the lid on and the ends of the ground that I should have covered hit the hot side of the fuse...
